Payment Terms

Momentum Precision Health is a cash-pay practice. Insurance is not accepted directly; however, we provide detailed superbills for potential reimbursement from your insurance provider. HSA and FSA cards are accepted for all services. Payment is due at the time of service unless otherwise agreed upon. Certain programs require a minimum commitment period as outlined in our pricing.

Cancellation & Refund Policy

We require at least 24 hours' notice for appointment cancellations or rescheduling. Late cancellations or no-shows may be subject to a fee. Refunds for prepaid services will be evaluated on a case-by-case basis. Monthly program fees are non-refundable once the billing cycle has begun.
